It’s almost always the first real question after we finish examining the scalp.

The patient is quiet for a moment, processing everything we’ve just explained – the over-harvested donor, the unnatural hairline, the scar tissue. And then it comes:

“Is this going to cost me more than the first time?”

It’s a fair question. Especially if the first experience already drained your savings and your confidence.

So let’s answer it honestly.

In most cases, yes. A repair procedure does cost more than a primary transplant.

But that answer only makes sense when you understand what a repair actually involves.

A first-time transplant is performed on untouched ground. The donor area is intact. The scalp hasn’t been altered. The blood supply is healthy. The geometry of the hairline hasn’t been distorted.

Repair surgery is different.

By the time someone walks in for a repair, the landscape has changed. There may be bulky grafts sitting in the wrong place. The hairline may be too straight or too dense. The donor area may already be thin.

In many cases, we can’t simply “add more hair.” We have to remove misplaced grafts first. Carefully extract them. Refine them under magnification. Re-implant them correctly. Sometimes we’re undoing before we redo.

That takes more time. More planning. More precision.

One of the most common issues in repair patients is donor depletion.

The back of the scalp is your lifetime reserve. When too many grafts are extracted too quickly, it becomes patchy. Scarred. Thinner than it should be. That limits future options.

If the scalp donor is compromised, we may need to use beard hair or other body hair. Body Hair Transplantation (BHT) is not a routine skill in most clinics. It is more time-intensive. It requires a team that understands how to balance texture, thickness, and density.

That expertise comes at a higher cost, because it protects what little donor resource remains.

Scar tissue doesn’t behave like untouched scalp. Blood flow may be reduced. Tissue can be firmer. Implantation angles become more critical.

In hair transplant correction surgery, graft survival matters even more than usual. There is very little margin for failure.

This is why we rely heavily on Direct Hair Transplantation (DHT) techniques in repair cases. By minimizing the time follicles spend outside the body and implanting them with controlled precision, we improve survival rates, especially in compromised areas.
