Of all the questions patients ask after a hair transplant procedure, washing hair after hair transplant surgery is the most common. In the first week your newly implanted follicles are still settling in, and pressure, rubbing, and hot water work against them.

When Can I Start Washing My Hair After a Hair Transplant?
Day 7. Not 24 to 48 hours, not Day 3, and not in the shower the morning after surgery.
Clinics online quote anything from two days to a week, so there isn’t one universally agreed washing schedule in hair restoration. Follow the clinic that performed your hair transplant surgery. At Eugenix, that means Day 7 for every patient.
Why the First Hair Wash Waits Until Day 7
Each graft sits inside a recipient slit created by your lead surgeon during the procedure. Over the next few days the skin seals those sites and the grafts anchor themselves. Until then the graft is held by a fine biological bond, not anything mechanical. Wash hair too early, and you can dislodge transplanted grafts before they are securely anchored.
There is a second reason. Small punctate red scabs of dried blood build up over the recipient area through the week, peaking around Day 6. By Day 7 they are mature enough to be softened and lifted away in one gentle head wash. Wash earlier, and they pull at grafts that are not ready.
Days 1 to 6: Keeping the Scalp Clean Without Washing
Avoid washing for six days, but that does not mean no care. Your scalp has an active routine through this part of the healing process.
Saline spray on the transplanted area
Spray normal saline over the transplanted area every 30 to 60 minutes through the day, though none is needed while you sleep. This keeps the skin moist, softens crusting, and eases the itching that starts around Day 3, when mild swelling may also appear. Cracks on the scalp come from dryness, and spraying is the answer.
Cleaning the donor area
The donor area is cleaned once daily with Betadine scrub, followed by Mupirocin ointment over the donor site. This runs for seven days and lowers infection risk. The donor site is usually healing well by Day 5, and the donor area is washed on Day 7 with the scalp.
Protecting the new grafts
Keep your hands clean and avoid touching the recipient area. Wear the surgical cap provided by the clinic when going out, and choose open-button shirts over t-shirts pulled over the head. Sleep with your head elevated on a few pillows, never face-down, which also helps swelling settle. Avoid direct sunlight on the scalp for seven days, and minimise sun exposure after.
Day 7: The First Head Wash, Step by Step
What shampoo to use after a hair transplant
Any mild shampoo works, with Dove a common choice. You do not need a special shampoo, a special medical shampoo, a medicated shampoo, or a baby shampoo, just a gentle shampoo without harsh chemicals. From Day 8, a regular shampoo with pH 5.5 to 7 suits daily use.
How to wash hair on Day 7
Set aside about half an hour. Use lukewarm water only, since hot water irritates the healing scalp at any stage after surgery.
- Wet the hair gently with saline water.
- Apply the mild shampoo.
- Foam it up slowly using only your fingertips, never the nails.
- Keep sprinkling saline every few minutes so the lather stays wet.
- Leave it lathered for about 20 minutes. The soak does the real work.
- Very gently rub the transplanted follicles with the fingertips to loosen scabs.
- Rinse with gentle pressure, using lukewarm water or saline. Avoid strong shower pressure and high-pressure water on the new grafts.
- Pat the scalp dry with a soft towel or let the hair dry in the air. Do not towel-rub.
The goal of the first wash after a hair transplant is cleanliness while protecting the grafts, not a spotless scalp.
If scabs remain after the first wash
Common, and part of normal healing. Repeat the wash the next day. Most patients need 2 to 3 consecutive washes before the recipient area is clear, so scabs are usually gone by Day 8 to Day 10. Never scratch or scrub them off.
Can I Touch My Grafts After 7 Days?
Only as much as the hair wash requires. From Day 7 you may gently rub the transplanted follicles with clean fingertips to release scabs. Otherwise, patients are advised to avoid touching or scratching. Transplanted grafts grow more secure through the second week, and by Day 10 the risk of dislodging one is gone.

How often to wash your hair
Wash your hair daily after the first wash. Daily washing for the next 7 to 10 days clears residual scabs and keeps the scalp clean. After that you can wash hair daily, on alternate days, or three times a week. Daily washing does not damage the scalp.
Water temperature and pressure
Lukewarm water, always, and rinse with gentle pressure. Keep high-pressure water off the transplanted area through the first 10 days. Hard water does not harm grafts, though it can irritate the skin.
Drying your hair
After you wash your hair, pat it dry or let your hair dry naturally. Avoid regular hair dryer use, since hot air damages the cuticle.
Hair products, colour and trimming
Avoid hair products until the hair transplant timeline below clears them.
Hair mousse: after 7 days, on the length only.
Hair fibres: after 10 to 15 days.
Trimming or a haircut: after 15 days.
Hair gel, serum or colour: after 30 days, ammonia-free colour preferred.
Keratin, straightening and oil: not recommended after hair restoration surgery.
Most patients get mild soreness and redness, which is completely normal and typically lasts 4 to 8 weeks.
Shedding, Shock Loss and What You See in the Shower
Around the one-month mark, many patients notice transplanted hair shafts coming away during a hair wash and assume they did something wrong. They did not.
Shedding begins about 30 days after the procedure and can continue to 90 days. The visible shaft falls, the root stays in place, and no washing routine causes or prevents it. Hair growth after a hair transplant begins in month 4, with full results at 10 to 12 months and 12 to 15 months for the crown.
Shedding and shock loss are different. Shedding affects transplanted hair and always happens. Shock loss affects native hair around the transplanted zone and may or may not happen, depending on how miniaturised it already was. Patients who see unexpected hair loss in an untreated area should report it to the team.
